Individuals (families), or their relatives, legal representatives (guardians, custodians), municipalities, non-governmental organisations, as well as other persons with the consent of the individuals concerned, may apply for placement in the institution by submitting an application to the local bodies of the Social Services Agency under the Ministry of Labour and Social Protection of the Population of the Republic of Azerbaijan, “DOST” centres, or the unified coordination centres for applications from family members of martyrs, military personnel injured in connection with the war, and persons with established disabilities.
• a document confirming the child’s identity;
• the decision of the TSEK determining the child’s disability;
• a medical certificate regarding the child’s health, indicating that the child is not a carrier of infectious diseases and providing information on their mental health;
• the relevant certificate regarding persons registered at the child’s place of residence;
• 2 photographs measuring 3 x 4 cm.
