Persons (families) in need of social services apply to the Social Services Agency to receive social rehabilitation services.
• a copy of the document confirming identity;
• a medical certificate regarding health, indicating that the person is not a carrier of infectious diseases and providing information on their mental health;
• the decision of the TSEK confirming the person’s disability;
• the relevant certificate regarding persons registered at the person’s place of residence;
• 1 photograph measuring 3 x 4 cm.
