Residential Social Services

Individuals (families), or their relatives, legal representatives (guardians, custodians), municipalities, non-governmental organisations, as well as other persons with the consent of the individuals concerned, may apply for placement in the institution by submitting an application to the local bodies of the Social Services Agency under the Ministry of Labour and Social Protection of the Population of the Republic of Azerbaijan, “DOST” centres, or the unified coordination centres for applications from family members of martyrs, military personnel injured in connection with the war, and persons with established disabilities.

For children deprived of parental care

· a document confirming the child’s identity;

· a copy of the identity document of the parent (guardian, custodian) (in the case of a child born out of wedlock – the relevant certificate issued by the district (city) registration office of the Ministry of Justice of the Republic of Azerbaijan);

· if the parent(s) have died, the death certificate or a certificate issued by the district (city) registration offices of the Ministry of Justice of the Republic of Azerbaijan confirming this;

· the relevant certificate regarding persons registered at the child’s place of residence;

· a copy of the document confirming the ownership rights of the parent(s) to real estate.

Depending on the reasons why the child is deprived of parental care, the following documents shall be attached to the application:

· if the parents are divorced – a copy of the divorce certificate and the parent’s consent;

· if one or both parents are imprisoned – the relevant court decision;

· if the parent(s) have a disability, the decision of the Medical and Social Expert Commission (hereinafter – TSEK) confirming this.

For children with disabilities who are not enrolled in education

· a document confirming the child’s identity;

· the decision of the TSEK determining the child’s disability;

· a medical certificate regarding the child’s health, indicating that the child is not a carrier of infectious diseases and providing information on their mental health;

· the relevant certificate regarding persons registered at the child’s place of residence;

· 2 photographs measuring 3 x 4 cm.

For children with disabilities who are enrolled in education

· a document confirming the child’s identity;

· a medical certificate regarding the child’s health, indicating that the child is not a carrier of infectious diseases and providing information on their mental health;

· the relevant certificate regarding persons registered at the child’s place of residence;

· 2 photographs measuring 3 x 4 cm.

For minors who are neglected or in a socially dangerous situation

· a written or verbal application from the minor;

· applications from the minor’s parents, relatives, other legal representative (guardian, custodian), municipalities, non-governmental organisations, as well as other persons with the consent of the relevant person, taking into account the wishes of a minor who has reached the age of 10, except in cases where taking the minor’s wishes into account would be contrary to their interests;

· an application from the local commission for the protection of children’s rights;

· a decision of the city or district police authority on placing minors in need of social rehabilitation in a specialised social rehabilitation institution intended for them.

For individuals (families) who, due to illness, disability or old age, lack the ability to care for themselves, as well as able-bodied relatives or legal representatives who can provide care and assistance

· an identity document;

· a medical certificate regarding health, indicating that the person is not a carrier of infectious diseases and providing information on their mental health;

· the decision of the TSEK confirming the person’s disability;

· the relevant certificate regarding persons registered at the person’s place of residence;

· 2 photographs measuring 3 x 4 cm.
